My copy of FFIV on the DS has an extra large old-style USK rating (German version of the ESRB). This is how all games printed between April and May 2009 look, but given the narrow time frame and the fact that this only applies to Germany I think this is kind of rare.
FFIII on the left to show what the ratings looked like before. After June 2009 they changed them to the modern rectangular boxes (but still big).
